When you apply to COLA’s Accreditation Program, you begin the process with instant access to tools that will help you document and maintain compliance with COLA requirements.

NOTE:  You must notify your state CLIA agency any time you need to change your CLIA certificate type. Failure to do so could result in a “nonaffiliated” laboratory status and CMS may terminate the CLIA ID.
